Melvin Alvarado Rivera Arrested for Workers’ Compensation Fraud


April 23, 2018. Command Investigations, Packard Claims Administration and Lion Insurance Company announce the arrest of Melvin Alvarado Rivera for workers’ compensation fraud.

He is charged with the following:

  • Florida Statute 440.105(4)(B) – Workers’ Compensation Fraud
  • Florida Statute 817.568(2)(A) – Criminal Use of Personal Identification Information Without Consent

Command Investigations began an investigation into Alvarado Rivera after he sustained an alleged work-related injury while installing a roof.

Command’s Claims Data Verification Program™ (CDVP™) determined that Alvarado Rivera provided false and misleading information to obtain workers’ compensation benefits after his injury.

Based on Command’s investigative findings, their Special Investigative Unit (SIU) referred the claim to the Florida Division of Insurance Fraud (DIF), as statutorily required.

Alvarado Rivera was arrested on April 13, 2018 and is currently being held in Brevard County, Florida in lieu of $4,000.00 bond.

All parties are presumed innocent until proven guilty.
