Joeluis Puertas Accepts Pre-Trial Diversion Agreement & Restitution Agreement

Joeluis Puertas Final Disposition
May 31, 2018. Command Investigations, Packard Claims Administration and Lion Insurance Company announce that Joeluis Puertas has accepted a Pre-Trial Diversion (PTD) program agreement.

Puertas is required to pay Packard Claims Administration $650.00 in enforceable restitution and he must successfully complete one year of a PTD program for the following charge:

  • Florida Statute 440.105(4)(B) – Workers’ Compensation

Command initiated an investigation into Puertas after he sustained a lower back injury while working as a courier.

During the claim investigation, Puertas provided a sworn statement claiming he had not suffered any prior accidents, injuries or received any medical treatments prior to his work-related accident.

The investigation determined that Puertas did receive prior medical treatments and had suffered previous injuries.

In fact, he had just returned to work after being out for multiple months from an unrelated injury.

Based on Puertas’ non-disclosure of a pre-existing injury, Command’s SIU Department was statutorily required to refer his claim for material misrepresentation.

Puertas was arrested on April 10, 2018 in Dade County, Florida.
