November 4, 2015. Command Investigations, Packard Claims and Lion Insurance Company announce a final disposition of $9,777.40 in restitution paid to the carrier and probation time served for Tara Herndon.
Herndon was ordered to pay enforceable restitution in the amount of $9,777.40, and also served probation time.
A referral was submitted to Florida’s Division of Insurance Fraud (DIF) as statutorily required based on material misrepresentation determined by the evidence collected during Command’s investigation. The evidence was submitted in conjunction with Herndon’s failure to report her earnings from a concurrent employer when submitting her DWC-19s.
Subsequently, DIF issued an arrest warrant, and on May 27, 2015, in Lake County, Florida, Tara Herndon was arrested for workers’ compensation fraud, violating F.S. 440.105(4b) – WORKERS COMPENSATION FRAUD FALSE STATEMENT IN SUPPORT OF CLAIM. She was released on a $15,000 bond.
Ultimately, a final disposition ordered Herndon to pay $9,777.40 in restitution to the carrier and serve probation time.
