December 27, 2016. Command Investigations, Next Level Administrators (NLA) and SUNZ Insurance Company (SUNZ) announce the arrest of Jose Luis Acosta-Quintero for insurance fraud.
Command began an investigation into Acosta-Quintero after he suffered an alleged work-related injury. During the investigation, Command’s Claims Data Verification Program™ (CDVP™) concluded that Acosta-Quintero provided a suspect Social Security Number in an attempt to secure workers’ compensation benefits.
Command’s CDVP™ established that the SSN provided by Acosta-Quintero was issued to a different individual who resides in the state of Florida.
As required under Florida Statutes 626.9891 and 440.105, Command’s SIU Department submitted a referral to the Florida Department of Financial Services (DFS), Division of Insurance Fraud (DIF) based on material misrepresentation.
Acosta-Quintero was arrested on December 19, 2016 in Orange County, Florida. He is charged with the following:
- Criminal Use of Personal Identification Information
- Workers’ Compensation Fraud. F.S. 440.105 – False Statements in Support of Claim
- Workers’ Compensation Fraud. F.S. 440.105 – False Identity in Support of Claim
- Grand Theft
- Fraudulent Use of Personal Identification Information
- Use False ID for Workers’ Compensation Claim less than $20K
He is being held in the Orange County Jail in lieu of $10,000 bail.
All parties are presumed innocent until proven guilty.
