Howard Williams Arrested for Insurance Fraud

Command initiated an investigation on Williams after he reported an alleged work-related accident. Through Command’s investigation, it was determined that Williams fabricated the accident and denied any prior medical history in an effort to obtain workers’ compensation benefits.

Williams past medical history was identified through Command’s iNQuest™ investigative medical canvasses program.  iNQuest™ revealed a previously non-disclosed past medical history in addition to specific lower-back treatments received prior to this alleged workers’ compensation accident.

Surveillance was also conducted by Command wherein video was obtained documenting repeated physical inconsistencies on behalf of Williams.

Based upon the total evidence gathered, Command’s SIU Department submitted a referral to the Florida Department of Financial Services (DFS), Division of Insurance Fraud (DIF) based on material misrepresentation, as statutorily required under Florida Statute 626.9891 and Florida Statute 440.105.

Williams was arrested on June 8, 2016, in Lake County, Florida, and charged with the following:

F.S. 440.105 (4b) – Workers’ Compensation Fraud

F.S. 817.234 (11a) – Fraudulent Insurance Claim

All parties are presumed innocent until proven guilty.
