November 18, 2016. Command Investigations, Next Level Administrators (NLA) and SUNZ Insurance Company announce Angelica Fonseca’s final disposition for insurance fraud.
Fonseca was ordered to pay $843.00 in enforceable restitution, serve 2 years of probation and complete a Pretrial Intervention Program (PTI).
Fonseca was arrested on August 4, 2016 and charged with the following:
F.S. 440.105 (4b9) – Fraud: Use False ID for WC Claim Less Than $20K
Command initiated an investigation into Fonseca after she reported a work-related accident. During the investigation, Command’s Claims Data Verification Program™ (CDVP™) ascertained Fonseca utilized a suspect SSN to receive medical treatment and workers’ compensation benefits.
It was determined that the Social Security Number provided by Fonseca did not appear to have been issued in the United States.
Based on evidence gathered during the investigation, Command’s SIU Department submitted a referral to the Florida Department of Financial Services (DFS), Division of Insurance Fraud (DIF) based on material misrepresentation, as statutorily required under F.S. 626.9891 and F.S. 440.105.
