June 4, 2015. Command Investigations, Packard Claims and Lion Insurance Company announce the arrest of Tara Herndon.
As Packard Claims’ and Lion Insurance Company’s SIU Department, Command Investigations initiated an investigation on Tara Herndon after she sustained an injured wrist on a jobsite. During a claims investigation, it was determined that Herndon submitted her DWC-19s and did not report her earnings from a concurrent employer. Command obtained information from this secondary employer stating Herndon had received pay from October 12, 2014 to January 3, 2015, which she failed to report.
Command’s SIU Department, working in conjunction with Packard Claims and Lion Insurance Company, gathered the evidence and submitted a referral to the Florida Division of Insurance Fraud (DIF) as statutorily required based on material misrepresentation. After review, DIF issued an arrest warrant for Herndon.
On May 27, 2015, Tara Herndon was arrested in Lake County, Florida, for insurance fraud, violating F.S. 440.105(4)(b)(2) – WORKERS COMPENSATION FRAUD FALSE STATEMENT IN SUPPORT OF CLAIM, F.S. 817.234(1)(a) – FALSE AND FRAUDULENT INSURANCE CLAIMS and F.S. 812.014(2)(c) – GRAND THEFT.
Herndon was released on $15,000 bond. Herndon’s trial is pending.
As a result of the SIU investigation and subsequent arrest, the approved claim savings have been estimated to be $27,500.
All parties are presumed innocent until proven guilty.
