June 1, 2016. Command Investigations, Packard Claims and Lion Insurance Company announce the arrest of Joel Olvera for insurance fraud.
Command initiated an investigation on Olvera after he sustained an alleged injury on a jobsite. Through Command’s Claims Data Verification Program™, it was determined that Olvera provided a suspect SSN to obtain workers’ compensation benefits.
The investigation verified the SSN provided by Olvera was issued to an individual in the state of Washington.
Based upon the evidence gathered, Command’s SIU Department, working alongside Packard Claims and Lion Insurance Company, submitted a referral to the Florida Department of Financial Services (DFS), Division of Insurance Fraud (DIF) based on material misrepresentation, as statutorily required under Florida Statute 626.9891 and Florida Statute 440.105.
Olvera was arrested on May 16, 2016, in Collier County, Florida, and charged with the following:
F.S. 440.105 (4) (b) 9 – Use False ID Worker’s for Comp Claim < $20,000
All parties are presumed innocent until proven guilty.
